ONLINE REGISTRATION FOR NEW SPRING PLAYERS IS NO LONGER AVAILABLE.
If you are interested in joining a CSC Team, please contact Tricia Brooks, CSC Executive Assistant. Once you have been accepted to a CSC Team for the Spring 2013 Season, please contact the CSC Registrar to be manually registered AND mail your registration fee to:

Registration fees: $440 for ages U10-U18; $300 for Academy players.

Your Team Manager will contact you to discuss uniforms and collect additional documents from you in order to complete the registration process.
These documents include:
- a copy of player’s birth certificate – this must be a state issued Birth Certificate, not a hospital certificate;
- a picture – a small “passport size” picture for the player’s card (similar size as a driver’s license picture). Some team managers may opt for a digital photo;
- Kentucky Youth Soccer Medical Release Form – This form must be notarized.;
- Photography Authorization and Release Form – This form must be signed; and
- Parent/Player Commitment Agreement - This form must be signed by player and parent.
