Registration

Submitting your payment, using the "Register Online" button below, will secure your player's position on his/her team and should be done within 24 hours of the posting of results for your team.  UPDATE:  If your son our daughter is playing up in age or has a scholarship, please contact Bill Massarone directly.  He will need to manually enter registration exceptions into the payment system.  For scholarship information, please click here (English) or here (Spanish).

Note that online registration is set to accept Credit Cards only (No checks or offline payments).  To complete the registration payment process please click on the button below.

Your team representative will contact you regarding the additional documents that will be needed within the next few weeks in order to complete the registration process.

These documents included:

• a copy of player’s birth certificate – this must be a state issued Birth Certificate, not a hospital certificate;
  

• a picture – a small “passport size” picture for the player’s card (similar size as a driver’s license picture).  Some team reps may opt for a digital photo;
  

• Kentucky Youth Soccer Membership Form – This form must be completed and signed;
  

• Kentucky Youth Soccer Medical Release Form – This form must be notarized.;
  

• Photography Authorization and Release Form – This form must be signed; and
  

• Parent/Player Commitment Agreement - This form must be signed by player and parent and indicates that you have read and agree to abide by the CSC Membership Handbook which is also available in PDF Format.
