Registration
At this time, all players should have completed and turned in their paperwork to their team representative.

The registration process included the following documents:

  • a copy of player’s birth certificate – this must be a state issued Birth Certificate, not a hospital certificate;
  • a picture – a small “passport size” picture for the player’s card (similar size as a driver’s license picture);
  • Kentucky Youth Soccer Membership Form – This form must be completed and signed;
  • Kentucky Youth Soccer Medical Release Form – This form must be notarized. A notary will be on hand at LCA on Saturday;
  • Photography Authorization and Release Form – This form must be signed; and
  • Parent/Player Commitment Agreement - This form must be signed by player and parent and indicates that you have read and agree to abide by the CSC Membership Handbook which is also available in PDF Format.

If your are attempting to make your Spring 2011 payment, the LeagueOne registration is, unfortunately, not configured to accept it.  Instead, please mail your payment to:

Make sure that you include your player's name and team name on the check.
